TK-401 Nozzle Weld Repair - Hot Work

Tank & terminal

Weld repair to the 6in outlet nozzle on TK-401. NFPA 51B hot work. Fire watch 30 minutes after work stops.

This is a worked example, not a procedure you can adopt. It describes equipment that does not exist, with illustrative tag numbers. 29 CFR 1910.147(c)(4) requires the employer to develop, document and utilise energy control procedures specific to their own machines, and (c)(6) requires the employer to inspect those procedures periodically. Neither duty can be delegated to a vendor or discharged by this document.
Other standards apply to this work.
  • 29 CFR 1910.252 and NFPA 51B — hot work. Requires a hot work permit, fire watch and gas testing in addition to the isolation shown here.
  • 29 CFR 1910.146 — if the repair requires entry into the tank.
A lockout-only document is misleading for work that also falls under another standard.

Isolation points

#Tag (illustrative)TypeDispositionWhat it does and whyDwell
10TK-401-DRAINDrainIsolate & lockTank drained, gas-freed and purged. Confirm on the gas-free certificate before any ignition source.
20TK-401-VAPORValveIsolate & lockVapour recovery line isolation valve. Prevents hydrocarbon vapour migrating back to the work.
30TK-401-INLET-BLINDBlindApply & verifyInlet line spectacle blind turned to BLANK. Positive isolation - a closed valve is not enough for hot work.
40HW-COMBUST-35FTBlockApply & verifyCombustibles within 35 feet removed or shielded with fire blankets. Includes above, below and behind walls.
50HW-OPENINGSBlockApply & verifyFloor and wall openings within 35 feet sealed. Sparks travel through gaps to hidden combustibles.
60HW-EXTINGUISHERBlockApply & verifyFire extinguisher rated 2-A:20-B:C staged within 30 feet, inspected and in date.
70HW-CYLINDERSBlockApply & verifyGas cylinders upright, chained and outside the hot work zone. Barrier required within 20 feet of flame.

What your site must supply

  • Your own asset and isolation-point tag numbers. The tags shown are illustrative and refer to equipment that does not exist.
  • The actual location of every disconnect, valve and vent on your equipment.
  • Measured stored-energy magnitudes and the dwell time required to dissipate them.
  • Your verification method for each point, and who is authorised to perform it.
  • Any additional energy sources present on your equipment and absent here.

Authorship and validation

This example was authored by Setyr for domain validation. It is not derived from any customer's controlled documents. It illustrates the elements 29 CFR 1910.147(c)(4)(ii) requires a procedure to contain — it does not certify compliance, which is a property of an employer's programme rather than of any document.
