What lockout looks like here

Wastewater is the industry where 1910.146 and 1910.147 are inseparable. Every tank entry is a confined space with an atmosphere that can change, and every pump, mixer and gate is an energy source that can start.

Small plants, small crews, one person doing entry, attendant coordination and the lockout. The record has to hold up when that person is also the one being asked about it.

Municipal budgets mean the equipment is old and the procedures are older.

How it goes wrong
Mixer isolated, influent gate not

The tank drains, the mixer is locked, and the influent valve upstream is still able to open on level control.

Entry permit and lockout kept in different binders

The confined-space permit is complete; the lockout for the same tank is a separate form nobody cross-referenced.

Rescue plan assumes the pump is off

It is not, because the lockout was on the local disconnect and the pump has a remote start.
